Are you interested in joining VHS Alumni Association?  Complete and submit the below form, then follow the payment instructions at the bottom of the page. 

After completing and submitting the above form, your lifetime membership fee of $25 can be paid one of two ways. Use the PayPal link below or mail your check to:
